This website was made to help youth move forward after leaving shelters or care. It connects you to real housing, food, health, and employment supports across Ontario. The Follow-Up Supports team at Covenant House Toronto works one-on-one with youth to build life skills, find stability, and stay connected to the community. Our goal is to make the next steps easier and give every young person a fair chance to grow and succeed.
